What this counts

Outlays for the Affordable Care Act's refundable premium tax credits, which lower the monthly premium of marketplace (Obamacare) plans, and cost-sharing reductions. Most of the credit is paid in advance directly to insurers and recorded as an outlay. From June 2026 the Treasury statement splits the line into a Treasury part and an HHS part; the two are added together here.

Figures are net outlays: payments made, minus any offsetting collections credited to the same accounts, as printed in the Monthly Treasury Statement. They are not adjusted for inflation. In the budget-function view it falls under health care and Medicaid.
